At the time of the release of the game Tyson was the undefeated and undisputed Heavyweight Champion of the World. This is reflected in the game play as Tyson is an extremely difficult challenge for even experienced video gamers; one uppercut can knock Mac down, his jabs are quick and deadly, and he is very tough to beat. His only weakness comes when he appears stunned and repeatedly 'winks' at the video game player, during which Little Mac's punches can often obtain 'stars' or knock Tyson down. Tyson's involvement in the game, and in the advertisements for it, helped make the game one of the most popular on the NES and was an early example of a video game endorsed by a professional athlete.

Although it is believed that Mike Tyson was removed from the game because of the rape of Desiree Washington, that didn't happen until 1991. He was essentially replaced with a recoloured sprite with a new face named [[Mr. Dream]]. Since Tyson had lost the title to James "Buster" Douglas by that point, Nintendo made no attempt to negotiate a new contract with him.

Mike Tyson made his video game debut in Mike Tyson's Punch-Out!! on the Nintendo Entertainment System as the final opponent of the game, whom Little Mac faces in the Dream Fight at the end. His uppercuts knock Little Mac down in one hit. Mike Tyson mostly does uppercuts and he will do punches after 90 seconds of the fight.
